Know what you're buying. Compounded medicines are mixed by a pharmacy for an individual prescription. FDA doesn't review them for safety, effectiveness or quality, and since the shortages ended in 2025 pharmacies can't routinely make copies of the brand drugs. Made by Eli Lilly and reviewed by FDA. Lilly sells Zepbound vials and the KwikPen for cash through LillyDirect, and the telehealth providers below fill through it at the same price. Medicare Part D members can get the Zepbound KwikPen for $50 a month through the Medicare GLP-1 Bridge.
Approved for weight management and for obstructive sleep apnea in adults with obesity. Self-pay only, through LillyDirect.
The autoinjector most insurance plans cover. Same medicine as the vial and KwikPen.
Approved for type 2 diabetes. Lilly says plainly that Mounjaro is not a weight-loss drug.

Questions
Tirzepatide is the active ingredient in Zepbound and Mounjaro. Zepbound is the brand approved for weight management and sleep apnea; Mounjaro is approved for type 2 diabetes. Compounded tirzepatide is not an FDA-approved product.
With coverage, the savings card can bring the pen to $25 a month. Paying cash, LillyDirect sells vials and the KwikPen for $299 at 2.5 mg, $399 at 5 mg and $449 at higher doses if you refill within 45 days.
The tirzepatide shortage ended in late 2024, so pharmacies can no longer make routine copies. A compounding pharmacy can still prepare it for a specific patient when a clinician documents a medical need.
