GLP-1 Price-Per-Pound-Lost: The New Value Metric for 2026

Monthly price is one comparison metric. Total 12-month cost is another. But the metric that's starting to actually predict patient satisfaction is price-per-pound-lost — total spend divided by weight loss achieved. Here's how to think about it.

Why the Metric Matters

Two programs at the same monthly price can produce very different weight-loss outcomes because efficacy depends on: medication itself (tirzepatide typically outperforms semaglutide), dose reached (higher doses produce more loss), retention (patients who stay on treatment lose more than patients who drop out), and program support (coaching and accountability improve adherence).

A cheaper program that produces less weight loss can cost more per pound than a pricier program that produces more. Comparing on price-per-pound-lost surfaces this reality.

How to Estimate the Metric

Price-per-pound-lost = total 12-month cost of program / expected pounds lost in 12 months. Where the Metric Breaks Down

Price-per-pound-lost is a rough estimation, not a precision measurement. Individual response varies enormously — some patients on semaglutide lose 25%, some lose 5%. Some programs have better real-world retention than others (a 40% dropout rate at $150/month is worse per-pound-lost than a 10% dropout rate at $250/month for patients who stay). And some patients get more value from bundled coaching that doesn't show up in the medication-cost-only calculation.
