The Model

Using published clinical trial data:

• Semaglutide average weight loss: ~15% of body weight over 68 weeks
• Tirzepatide average weight loss: ~20-25% of body weight over 72 weeks
• Reference patient: 240 lbs starting weight
• Treatment period: 12 months

At 15% loss, our reference patient loses 36 lbs on semaglutide. At 22.5% (midpoint for tirzepatide), they lose 54 lbs.

The Surprising Finding

Here's what the data reveals: tirzepatide's cost per pound lost is often comparable to or better than semaglutide's, despite its higher monthly price. At GobyMeds, tirzepatide's CPL ($30/lb) actually beats semaglutide's ($33/lb) because the greater weight loss more than offsets the higher monthly cost.

This doesn't mean tirzepatide is universally the better value — individual results vary enormously, and you might respond better to semaglutide. But it does mean that comparing providers purely on monthly cost misses half the picture.
