When a provider quotes you $179 per month for compounded semaglutide, that number could mean two different things. It might include everything — medication, shipping, and telehealth consultations. Or it might cover only the medication, with a separate telehealth visit fee charged each time you see a provider. The difference can add $50 to $150 per month to your actual cost, and most comparison sites do not distinguish between the two models.
The Two Billing Models
All-inclusive pricing. The monthly rate covers medication, pharmacy dispensing, shipping, and all provider consultations. Whether you message your provider twice or have a video visit, the cost does not change. This is the simpler model and currently the more common one among compounded GLP-1 providers.
Decoupled pricing. The medication has one price, and telehealth visits have a separate fee. The initial consultation might cost $49 to $99, with follow-up visits at $29 to $75 each. Some providers require follow-up visits at specific intervals (monthly, quarterly); others bill only when you request one.
💡 Why this matters
A provider advertising semaglutide at $99/mo with a mandatory $49 monthly follow-up is actually a $148/mo program. Providers advertising at $179/mo all-inclusive are $179/mo. The headline price comparison is misleading without this distinction.
Which Providers Bundle vs. Decouple
| Provider | Model | Visit Fee | Visit Frequency |
|---|---|---|---|
| Embody | All-inclusive | $0 (included) | As needed |
| Gala | All-inclusive | $0 (included) | Provider-initiated |
| Yucca Health | All-inclusive | $0 (included) | Monthly check-in |
| Telos Rx | Visit included | $0 (included) | Cancel anytime |
| MadeMed | Subscription-based | Included in plan | On request |
| Sesame Care | Visit fee separate | Varies | Per consultation |
| Trimi | All-inclusive annual | $0 on annual plan | Per titration step |
The all-inclusive model dominates the compounded GLP-1 market because it simplifies marketing — one number, one monthly charge. Decoupled models are more common among providers that also offer brand-name medications, where the consultation fee covers the prescribing service and the medication cost is handled separately through insurance or manufacturer coupons.

When Decoupled Pricing Works in Your Favor
Decoupled models are not inherently worse. In some scenarios they cost less:
Stable maintenance patients. If you have been on the same dose for 6+ months and only need a provider check-in quarterly, a low medication cost plus infrequent visit fees can undercut a higher all-inclusive rate. A patient paying $99 per month for medication plus $49 quarterly for visits ($99 + ~$16 per month amortized) pays $115 per month — less than many all-inclusive programs at $179 and above.
Multi-condition patients. If you see a telehealth provider for GLP-1 and also for other conditions, a per-visit fee structure might cover multiple prescriptions in a single consultation, effectively splitting the visit cost across treatments.
Self-managing patients with PCPs. If your primary care provider handles your health monitoring and you only need the telehealth service for the prescription itself, minimal visit frequency works in your favor under a decoupled model.
⚠️ Watch for hidden visit requirements
Some providers advertise low medication prices but require monthly video consultations that carry separate fees. Before enrolling, ask: Are follow-up visits required? How often? What does each visit cost? Can I do asynchronous check-ins instead of video calls?
The True Monthly Cost Formula
To compare providers accurately across billing models, calculate the true monthly cost:
True Monthly Cost = Medication/mo + (Visit Fee × Required Visits/mo) + Membership Fee/mo + Shipping/mo
For all-inclusive providers, this is simple — the advertised rate is the true monthly cost. For decoupled providers, you need to know the visit frequency requirement to calculate the real number. On our comparison page, we show the all-in monthly cost whenever possible.

Questions to Ask Before Enrolling
Before choosing any GLP-1 provider, get clear answers to these questions:
1. Does my monthly rate include all consultations? If not, what does each visit cost and how often are they required?
2. Is there a separate membership or platform fee? Some providers charge $49 to $99 per month on top of medication costs for "membership" or "platform access."
3. What happens if I need an unscheduled visit? Nausea, injection site reactions, and dose adjustment questions can arise between scheduled check-ins. Know whether those contacts cost extra.
4. Is shipping included? Most compounded providers include shipping. A few charge $9.99 to $15 per shipment for expedited or cold-chain delivery. Over 12 months, that adds $120 to $180.
💡 Bottom line
All-inclusive pricing is easier to budget and compare. Decoupled pricing can be cheaper if you're a stable, low-touch patient. Either way, calculate the true monthly cost before enrolling — not just the headline medication price.
