The New Multi-Month Discount Standard: 3-Month, 6-Month, 12-Month Pricing Compared
Multi-month prepay has become the dominant discount structure across compounded GLP-1 providers in 2026. Here's how the 3-month, 6-month, and 12-month tiers compare, when each makes sense, and what to watch for in the terms.
The Standardization
Two years ago, provider pricing was almost entirely monthly. Today, multi-month prepay is offered at nearly every credible compounded GLP-1 provider. The typical structure: standard month-to-month at headline rate, 3-month prepay at 10-15% discount, 6-month prepay at 15-20% discount, 12-month prepay at 20-25% discount.
The provider motivation is clear: prepaid patients have committed cost and lower cancellation rates, which improves the program's unit economics. The patient motivation is proportional savings — a 20% discount on a $250/month program is $600/year.

When 3-Month Fits
Three-month prepay fits patients who: have completed initial titration and know the medication works for them, want moderate savings without long-term commitment, and value the ability to reassess quarterly. The 10-15% discount tier is enough to matter without locking in long-term.

When 6-Month Fits
Six-month prepay fits patients who: have several months of steady-state experience and are confident in ongoing treatment, want deeper savings, and have the cash flow to prepay half a year. The 15-20% discount tier is meaningful and the commitment window is still shorter than a full year.

When 12-Month Fits
Twelve-month prepay fits patients who: are committed to a full year of maintenance treatment, want the deepest available discount, and have confidence in the provider's operational stability. The 20-25% discount tier saves the most, but it also commits the most — and if the provider has continuity problems mid-year, the prepaid cost is at risk.

What to Verify Before Prepaying
Before any multi-month prepay: verify the refund policy for early cancellation (prorated? partial? none?), check the provider's regulatory status and warning-letter history, understand what happens if the medication is discontinued mid-cycle for clinical reasons, and confirm that the price is locked at the prepaid rate for the full period. Providers that answer all four clearly are safer prepay bets than providers that obscure any of the four.
