Foundayo (Orforglipron) Hits the Market

Eli Lilly's oral GLP-1 pill — the first that can be taken any time of day without food or water restrictions — launched via LillyDirect in April 2026 and is now widely available at pharmacies. Self-pay pricing: $149/month for the 0.8 mg starter dose, $199 for 2.5 mg, and $299 for maintenance doses (5.5-17.2 mg). With commercial insurance and savings card, as low as $25/month.

This is significant because Foundayo is a small-molecule drug (not a peptide), meaning it can't be compounded the same way semaglutide can. It's also not affected by the FDA compounding crackdown since there's no compounding pathway for orforglipron.

Medicare GLP-1 Bridge: 39 Days Away

The Medicare GLP-1 Bridge launches July 1, 2026. Eligible Medicare Part D beneficiaries will access Wegovy (injection and oral tablet), Foundayo, and Zepbound KwikPen at a $50/month copay. This is the first time Medicare has ever covered weight loss medications. Eligibility requires BMI ≥35, or BMI ≥27 with comorbidities. The program runs through December 31, 2027, when the longer-term BALANCE Model takes over.

Novo Nordisk 2027 Price Cuts Confirmed

Effective January 1, 2027, the list price of Wegovy, Ozempic, and Rybelsus drops to $675/month — roughly 50% off Wegovy's current list price and 35% off Ozempic. Self-pay prices through NovoCare remain unchanged. This primarily benefits patients on high-deductible plans whose out-of-pocket costs are linked to list price.

FDA Compounding Crackdown Update

The regulatory pressure on compounded GLP-1s continues to intensify. The FDA declared semaglutide and tirzepatide shortages resolved, and enforcement discretion periods for compounding pharmacies have ended. The FDA has proposed excluding semaglutide, tirzepatide, and liraglutide from the 503B Bulks List, which would permanently bar outsourcing facilities from bulk compounding these drugs.

Courts have upheld the FDA's position, with legal challenges from the Outsourcing Facilities Association failing to secure preliminary injunctions. More than 455 adverse events have been reported for compounded semaglutide and 320+ for compounded tirzepatide.

What this means for patients: compounded GLP-1s from licensed pharmacies remain available from many telehealth providers today, but the regulatory window is narrowing. If you're currently on compounded medication, having a transition plan to brand-name or FDA-approved alternatives is prudent.
