Weight-loss medication marketing is structured around a journey with an endpoint. The clinical evidence does not support that framing, and neither does the approved labelling.
What the approval language actually says
Foundayo is approved for use in combination with a reduced-calorie diet and increased physical activity to reduce excess body weight and maintain weight reduction long term in adults with obesity, or adults with overweight in the presence of at least one weight-related comorbid condition.
The phrase to sit with is maintain weight reduction long term. That is an indication for ongoing therapy, written into the approval.
If a medication is chronic therapy, the relevant financial question is annual cost for an indefinite period, not total cost of a course. Almost every GLP-1 price comparison in circulation — including most of ours until we corrected it — implicitly models a single year.
What discontinuation looks like
Across the GLP-1 class, extension and withdrawal studies have consistently shown substantial weight regain after stopping. The mechanism is not mysterious: these drugs work on appetite regulation while they are present, and the underlying physiology reasserts itself when they are not.
This is the single most important thing to understand before starting, and it is the thing least often stated plainly in provider marketing.

The compounded-supply problem is a year-two problem
If this is chronic therapy, then supply durability is not a minor consideration — it is a core one. On April 30, 2026 the FDA proposed removing semaglutide, tirzepatide, and liraglutide from the 503B Bulks List, finding no remaining clinical need for outsourcing facilities to compound them. Novo Nordisk has said it has filed 132 suits across 40 states, winning 44 permanent injunctions.
A compounded provider that saves you $50 per month and disappears in month fourteen has not saved you anything, because a gap in therapy means re-titration and, given the discontinuation evidence, potentially regain in the interval.

- Does that price change after twelve months for any reason?
- If I reduce to a lower maintenance dose, does the price drop?
- Does my insurance coverage or savings card eligibility renew annually, and on what terms?
- If you stop offering this product, what is your transition plan for existing patients?
Do not evaluate a GLP-1 program on the price of month one. Evaluate it on the price of month twenty-four, at your maintenance dose, from a supplier that will still exist.
A note on what we are not saying
None of this is an argument against GLP-1 therapy. The trial evidence for efficacy in weight reduction is substantial, and obesity carries real health consequences that these drugs measurably improve. Orforglipron is also being studied for obstructive sleep apnea and hypertension in adults with obesity, and a type 2 diabetes application is planned based on the phase 3 ACHIEVE program.
The argument is only about honest budgeting. A medication that works and that you can afford indefinitely is a good decision. A medication that works and that you have budgeted for twelve months is a decision you will have to revisit under pressure, at exactly the moment when stopping is most costly.
